An Evaluation on the Therapy and the Prphylaxis with Netilmicin(Netilin^(¢ç)) in Cardiothoracic Surgery


Abstract
This study was performed to investigate the efficacy of Netilmicin used prophylactica ly in heart and thoracic surgery and evaluate its efficacy and toxicity in the treatment of various patients undergoing cardiothoracic surgery, when used alone or in combination w¢¥th other antibiotics.
During Oct. 1984 and May 1985, 28 patients were included into this study. Patients w re divided into two groups; a) prophylatic group in 10 patients with various clinical diagno is and b) therapeutic group in 18 patients with different clinical diagnosis.
We used Netilmicin in combination with Cafazolin, Carbenicillin or Cefoperazone in h therapeutic group b) and Penicillin G or Cefamezin in 4 cases of the prophylatic group a) as a rule not longer than 4-6 days. The average treatment period of Natilmicin was 16 .days with a range of 6-30 days.
On admission the treatment group revealed bacteriologically Klebsiella pneumoniae, udomonas aeruginosa, staphylococcus aureus. All bacteria were found to be sensitive to Netilmicin.
Not one of the patient revealed any signs of nephrotoxicity or ototoxicity.
The clinical evaluation was carried out using a 4 point scale evaluating wound healing,
general status and condition, laboratory results, and signs of complications.
We found; 22 cases with complete resolution, 4 cases with major improvement, 2 cases
-with indeterminate results, and no failure was reported.
In summary 79% revealed complete resolution and 14% major improvement, that means ,our result, revealed a 93% efficacy rate.
This favourable results regarding toxicity together with a 93% efficacy rate clearly indicate that Nitilmicin is a highly effective antibiotic with a broad spectrum of activity and very low toxicity.
